Registration fees by date.
FACUG or APCUG member attendees
- $75 per person if postmarked or online by November 1st, 2011.
- $95 per person after that date.
- $25 per second person in cabin if postmarked or online by November 1st, 2011.
- $45 per second person in cabin after that date.
Non-member group attendees
- $100 per person if postmarked or online by November 1st, 2011.
- $120 per person after that date.
- $25 per second person in cabin if postmarked or online by November 1st, 2011.
- $45 per second person in cabin after that date.
NO REFUNDS AFTER SEPTEMBER 1, 2011. Substitution of attendee fee $15.

About the OnLine Form
After
this application form has been submitted you will have the opportunity to pay
online for the Conference via PayPal. If you choose not to do this please use the PDF Form.
About PayPal: To pay with PayPal you will be asked to create an account with
them if you do not already have one. It is very simple to do and then you can
use your Credit Card to pay for Registration.
You may register up to 2 people with this form.
The full information of the person filling in the form and paying is required. If registering for more than 1 person fill in their information in the additional attendee sections.
If you are paying for someone else and your name is not on the form, PLEASE make a note in the Special Needs section of the form.
Payment Instructions
The PayPal buttons you will see let you pay for your Conference Registration.
If you are registering for Members ONLY or Non Members ONLY _You need to select the appropriate Button.
You can then enter the number of Registrations you are paying for at Check Out.
Combination Registration:
If you are paying for a combination of Members and NonMember Registrations, use the Combination Payment button.
You can then enter the Total Amount at PayPal Check Out.
You will be asked to supply your Name and Phone No. this will confirm your identification and if the Treasurer finds an error she will contact you.
